Facility Scheduling Form
    If you would like to schedule an event for your organization/group in our parish facility, please fill out the information below and your event will be included on the parish calendar. If there is a scheduling conflict, you will be contacted. If you have any questions, please call 630-543-1754. If you would like the parish maintenance staff to set up your space, please fill out a facility set-up form. The completed form can be faxed to 630-543-9858, e-mailed to [email protected], or dropped off in the rectory at 1223 W. Holtz Avenue in Addison.
